The artwork depicts how those seated in the higher hierarchy can forcefully blind and physically hurt those who resist. With blinded eyes, blooded scars and ears from hearing nothing but the truth, the only way to intimidate the resistance is to forcefully blind them from it, but this can't and will not ever prevent them from their voices.
The man represents a red-tagged individual that has no choice but to consume the government's control over him.
